Job summary
Gentiva is seeking a dedicated Hospice RN / Case Manager to provide hands-on nursing care in patients' homes and other settings. You will perform comprehensive assessments, develop and update plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, personalized end-of-life care.

Qualifications
- Registered Nurse with active NY license
- Experience in hospice or palliative care preferred
- CPR certification and driving license required
Responsibilities
- Be the primary contact for patients and families guiding them through hospice care
- Deliver and document skilled nursing care in patients' homes or other settings
- Perform initial and ongoing patient assessments and adjust care plans
- Collaborate with an interdisciplinary team to refine care plans
- Provide emotional, physical, and spiritual support to patients and families
- Educate caregivers for safe patient care and comfort
- Communicate updates to the care team and ensure timely interventions
- Maintain accurate clinical documentation and comply with infection control standards
- Support quality improvement and scheduling initiatives for consistent care
