Job summary

You will collaborate with physicians, social workers, chaplains, and other professionals to support symptom management, end-of-life care, and caregiver education while maintaining regulatory compliance and high-quality patient care.

Qualifications

• RN licensed in the state of employment.
• Minimum 1 year RN experience in hospice/home health or related field.
• CPR certification required.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic, with education for patients and caregivers.
• Deliver nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans with visit frequency and risk identification.
• Coordinate care with IDG and physicians to refine care plans.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Communicate changes in condition to the care team for timely interventions.
• Support continuity of care across home health, palliative, and hospice referrals and transitions.
• Document timely clinical records (assessments, orders, progress notes, care plans).
• Maintain relationships with IDG and involve team as needed for outcomes.
• Participate in on-call and patient support activities as applicable.
