Qualifications

• RN licensure in state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year as an RN in relevant settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Coordinate care with IDG and physicians.
• Educate patients and caregivers on end-of-life care and medications.
• Document care plans and visits accurately.
