Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings, coordinating with physicians, social workers, and the broader care team. You will assess, plan, and implement individualized plans of care, ensuring comfort, dignity, and support for patients and families.

The role emphasizes education, symptom management, timely documentation, and collaboration across disciplines to improve quality of life for patients facing life-limiting illness.

Qualifications

• Current RN license to practice in Oklahoma
• CPR certification
• Valid driver’s license and auto insurance
• Minimum 1 year RN experience in hospice, home health, or related setting

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings; provide education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams including physicians and nurses.
• Document timely clinical records and maintain accurate progress notes and care plans.
