Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patients and families through the hospice journey. You will deliver hands-on nursing care in patients' homes or alternative living settings, conduct assessments, and collaborate with an interdisciplinary team to tailor care plans.

Qualifications

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• Current automobile insurance and valid driver's license.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver skilled nursing care based on each patient’s Plan of Care in home settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones; offer education and bereavement guidance.
• Educate caregivers on safe, confident home care practices.
• Communicate updates to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent care.
