Optimal Care is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative geriatric care in homes, assisted living, and facilities within Grand Rapids, MI. The role emphasizes time, patient relationships, and autonomy within a collaborative practice.

You will perform comprehensive evaluations, create treatment plans, prescribe medications, and coordinate with physicians and caregivers to improve comfort, function, and quality of life.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation with valid driver's license and auto insurance coverage.

Responsibilities

• Provide primary and consultative geriatric care at home and facilities.
• Develop and manage individualized treatment plans and monitor medications.
• Coordinate with physicians, caregivers, and the care team to prevent hospitalizations.
