Job summary

Optimal Care is seeking a Nurse Practitioner or Physician Assistant to deliver primary and consultative geriatric care, often in patients' homes or care settings. The role emphasizes time, connection, and meaningful patient relationships, with autonomy within a collaborative practice framework.

You will evaluate patients, develop treatment plans, prescribe medications, and coordinate with physicians, caregivers, and the interdisciplinary team to improve comfort, function, and quality of life.

Qualifications

• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians and interdisciplinary team members.
• Serve as a key clinical decision-maker and help patients and families understand diagnoses and manage chronic illness.
