Job summary

Optimal Care in Michigan seeks a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to deliver high-quality, patient-centered care that improves comfort, function, and life.

You'll conduct evaluations, develop treatment plans, prescribe medications, and coordinate with physicians, caregivers and the care team to reduce hospitalizations and enhance outcomes.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Conduct comprehensive medical evaluations for geriatric and medically complex patients.
• Develop and manage individualized treatment plans; prescribe and monitor medications.
• Coordinate with physicians, caregivers, and the interdisciplinary team to avoid hospitalizations.
• Operate within a collaborative practice framework to support patient-centered care.
