Job summary

Optimal Care - Evergreen in Grand Rapids, MI seeks a Nurse Practitioner or Physician Assistant to deliver primary and consultative geriatric care across home health and care settings. You will diagnose, treat, and manage chronic conditions with autonomy within a collaborative practice that values patient relationships and outcomes.

This role requires frequent travel within Holland, Hudsonville, Zeeland and surrounding areas, a current license, and a commitment to compassionate, high-quality care

Qualifications

Master's or doctoral degree as a Board-Certified NP or PA.
Current State License as NP or PA.
Reliable transportation, valid driver's license, and valid automobile insurance.

Responsibilities

Conduct comprehensive medical evaluations and develop individualized treatment plans.
Prescribe and monitor medications and coordinate with physicians and the interdisciplinary team.
Serve as a key clinical decision-maker to help patients and families understand diagnoses and manage chronic illness.
