Job summary

Optimal Care is seeking Nurse Practitioners or Physician Assistants to deliver primary and consultative care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on high-quality, patient-centered care. You will conduct comprehensive evaluations, manage treatment plans, and coordinate closely with physicians and caregivers.

Roles involve daily travel within the Grand Rapids, Holland, Hudsonville, Zeeland service area, Monday–Friday, 8 a.m.–5 p.m.,

Qualifications

• Must hold a master’s or doctoral degree and be board-certified NP or PA.
• Current state license as NP or PA.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in homes or care settings.
• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Prescribe and monitor medications and coordinate with physicians and care team.
• Serve as a clinical decision-maker within a collaborative practice framework.
