• Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients, often in home or care settings.
• Develop and manage individualized treatment plans, prescribe and monitor medications, coordinate with physicians and interdisciplinary team members.
• Serve as a key clinical decision-maker and help patients and families understand diagnoses and manage chronic illness.
