Job summary

Optimal Care - Evergreen in Gaylord, MI is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric patients. This role offers a flexible schedule with no weekends or holidays and emphasizes patient-centered care.

Qualifications

• Minimum 2 years clinical experience as a Nurse Practitioner or Physician Assistant.
• Adult, Family, or Gerontology certification is preferred.
• Experience with geriatric patients is a plus.

Responsibilities

• Provide primary and consultative care to geriatric and medically complex patients.
• Conduct comprehensive medical evaluations and develop individualized treatment plans.
• Coordinate closely with physicians and interdisciplinary team members.
