Job summary

Health Connect America in Alabama is seeking a Family Development Specialist to support clients and families by developing treatment plans and facilitating therapy within home-based settings.

You will coordinate care with the treatment team, provide individual, group, and family interventions, and actively recruit referrals while ensuring compliance with COA standards and regulatory requirements. A bachelor's in a human services field and at least two years' experience are required.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions, and skill-building as outlined in the treatment plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of interactions, treatment plans, and progress notes per policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
