Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordination Supervisor (unlicensed) in Ross County, OH. The role emphasizes high-fidelity wraparound care coordination for children and families, with focus on quality, collaboration, and community resources.

Eligible for a $5,000 hiring bonus, salary range $52,609.67–$63,282.33, and a comprehensive benefits package. The supervisor will lead a care coordination team, provide in-home and community training, ensure documentation

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology, or another related field.
• LSW/LPC preferred.
• Three years of experience providing coordinated supportive services/care coordination services.
• Previous supervisory experience strongly preferred.
• Experience working in the behavioral health field, preferably with multi-system youth or individuals with autism and/or developmental disabilities.
• Working knowledge of local, state, and federal regulations for behavioral health, developmental disabilities, and autism.
• Excellent communication and organizational skills.
• Experience working with Electronic Health Record (EHR) systems.
• Ability to operate video calls/meetings and maintain high-speed internet.

Responsibilities

• Supervise and oversee care coordination team members through meetings, supervision, and regular communications.
• Provide training for direct care coordinators in home and community settings.
• Support staff through individual and group conferences to improve skills and problem-solve cases.
• Promote teamwork and recognition among staff.
• Ensure completion of care plans, assessments, and reporting in accordance with standards.
• Coordinate on-call rotations and fidelity review processes.
• Collaborate with systems and partners to ensure high-quality services and family voice.
