Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordination Supervisor to lead the wraparound care coordination team in Ross County, OH. The role emphasizes high fidelity, collaboration with partners, and ensuring quality services for children and families.

Qualifications

• Bachelor’s or master’s degree in social work, education, counseling, psychology or related field preferred.
• LSW/LPC preferred.
• Three years of experience providing coordinated supportive services/care coordination.
• Previous supervisory experience strongly preferred.
• Experience in behavioral health field required, preferably with multi-system youth, or autism/developmental disabilities.
• Knowledge of high-fidelity wraparound services preferred (training post-hire).
• Excellent communication skills, both verbal and written.
• Excellent organizational skills; ability to prioritize multiple tasks.
• Knowledge of local/state/federal regulations for behavioral health and disabilities.
• Familiarity with wraparound planning processes (OhioRISE ICC/MCC).
• Experience with Electronic Health Records and related systems.

Responsibilities

• Supervise and oversee activities of the care coordination team.
• Provide training for direct care coordinators in home and community settings.
• Provide routine support and skill development through conferences and case analysis.
• Promote team recognition and a collaborative work culture.
• Complete and maintain high-fidelity wraparound training credentials.
• Ensure Medicaid-ready documentation and care plans are completed.
• Monitor on-call rotation and fidelity review processes.
• Assist in staff selection and onboarding processes.
