Qualifications

Responsibilities

• Provide comprehensive medical and non-medical case management to clients living with HIV/AIDS under the Ryan White Program.
• Conduct client assessments and develop individualized care plans.
• Coordinate community resources and services; maintain detailed records.
• Perform HIV testing and counseling; work as part of an interdisciplinary team.
