Job summary

The State of Florida, Department of Health, seeks a Senior Human Services Program Specialist to serve as a Ryan White Case Manager. Responsibilities include intake, financial assessment, CAREWare data entry, and eligibility determinations for HIV/AIDS services.

Typical duties involve client interviews, chart creation, documentation, and coordinating with the DOH-Volusia clinical team, with emphasis on confidentiality and compliance with state and federal regulations.

Qualifications

• Knowledge of HIV/AIDS program.
• Knowledge of social, economic or healthcare services.
• Ability to communicate effectively verbally and in writing.
• Ability to maintain confidentiality.
• Must be willing to physically come in the office to perform duties.
• Must have authorization to work in the United States without employer sponsorship.

Responsibilities

• Conduct face-to-face interviews to determine eligibility for Ryan White core services.
• Maintain client data in CAREWare, HMS, ADAP, FLMMIS, WAGES and other programs.
• Create client charts with forms, releases, progress notes and demographics.
• Complete monthly, quarterly and annual reports to supervisor.
• Coordinate care with the DOH-Volusia clinical team.
• Provide referrals and education on health literacy, adherence and retention in care.
• Provide staff training in Ryan White Case Management and other subprogram areas.
• Submit programmatic reports monthly to program manager/coordinator.
