Virginia Commonwealth University (VCU) School of Medicine in Richmond seeks a Ryan White Case Manager to coordinate care for clients with HIV, perform assessments, referrals, and care planning, and maintain compliant documentation.

The role emphasizes data entry in CAREWare/EPIC, participation in quality improvement, and ongoing professional development. This is a 40-hour onsite position in the Pediatrics department under the MBU unit.

Qualifications

• 3 years of Case Management experience in healthcare delivery.
• Knowledge of Medicare, Medicaid, and other eligible programs.
• Knowledge of HIPAA and privacy practices.
• Excellent communication and interpersonal skills.
• Strong time management and organizational abilities.
• Ability to work independently and in teams.
• Proficiency in MS Office and data-entry processes.
• Experience with HIV/AIDS related issues and confidentiality.

Responsabilités

• Coordinate case management to improve access to HIV care and services.
• Perform assessments, referrals, and care planning with multidisciplinary teams.
