Job summary

Hope & Help Center of Central Florida is seeking a compassionate Referral Specialist to join our Ryan White Case Management team in Orlando. You will help individuals access HIV medical care and support services, conduct eligibility screenings, coordinate referrals, and assist clients in navigating benefits and services to stay connected to care.

Qualifications

• Associate’s degree in social work, psychology, human services, public health, or related field.
• At least one year of experience in health or social services, patient navigation, or related role.
• Strong communication, organization, documentation, and client service skills.
• Ability to manage competing priorities, deadlines, and client needs.
• Valid Florida driver’s license with a clean driving record.
• Ability to work effectively with individuals from diverse backgrounds.

Responsibilities

• Conduct initial screenings for Ryan White program eligibility and assist with enrollment and recertification.
• Verify required documentation including insurance, income, residency, and HIV status.
• Connect clients with HIV medical care, medical case management, housing, food, transportation, and other services.
• Track referrals and follow up to confirm client connections to needed services.
• Help clients understand and navigate Ryan White system, Medicaid, ADAP, PAPs and other resources.
• Identify and reduce barriers involving transportation, documentation, insurance, and access to care.
• Maintain accurate documentation of referrals, contacts, eligibility, and outcomes.
• Collaborate with case managers and partners to coordinate client care.
