Job summary

Virginia Commonwealth University School of Medicine in Richmond, VA seeks a Ryan White Case Manager to coordinate access to medical and support services for clients living with HIV. The role involves assessments, care planning, referrals, and documentation in CAREWare and EPIC, with activities in quality improvement and community outreach.

Qualifications

• Minimum 3 years of case management experience in a healthcare delivery system.
• Knowledge of Medicare, Medicaid and other state/federal assistance programs.
• HIPAA knowledge and ability to maintain confidentiality.
• Excellent customer service and interpersonal skills.
• Excellent time management and organizational skills.
• Ability to work independently and in teams.
• Proficient in MS Office and data entry programs.
• Experience handling confidential health information and HIV/AIDS issues.

Responsibilities

• Case management including eligibility screening, assessments, referrals and care planning.
• Documentation in CAREWare and EPIC per policy.
• Participate in Quality Management/Quality Improvement activities.
• Attend supervisor meetings and trainings; participate in community events.
• Engage in professional development activities through the Ryan White CE program.
