Job summary

Prisma Community Care seeks a Ryan White Case Manager to deliver comprehensive case management for people living with HIV in the Phoenix area. You will assess needs, develop service plans, and coordinate with internal and external programs to support self‑management of care.

The role requires experience with diverse populations, strong documentation, and knowledge of HIV resources, insurance navigation, and care coordination within a supportive team.

Qualifications

• Bachelor's degree in social work or related field, or 4+ years of case management experience.
• Ability to communicate with culturally diverse populations effectively.
• Knowledge of HIV-related barriers, insurance, housing, and care access.
• Understanding of gender identity and LGBTQIA+ community issues.

Responsibilities

• Conduct psychosocial assessments and eligibility screenings for Ryan White clients.
• Develop individualized service plans and monitor progress with regular re-evaluations.
• Close gaps in medical care and support adherence to treatment and visits.
• Manage a caseload with measured billable time for Ryan White program.
• Document contacts and referrals in EMR within required timeframes.
• Provide information, referrals, and insurance navigation support.
