Job summary

Hamilton Health Center Inc. is seeking a Medical Case Manager to provide HIV-focused case management through the HOPE program under Ryan White Part B. You will help clients access medical care, promote treatment adherence, and support continuity of care while ensuring compliant documentation.

You will work with an interdisciplinary team to deliver patient-centered services and participate in outreach activities, continuing education, and community engagement initiatives.

Qualifications

• Knowledge of case management principles in healthcare.
• Ability to learn program-specific data systems and databases.
• Strong communication and engagement with diverse populations.

Responsibilities

• Provide specialized case management for individuals living with HIV.
• Facilitate access to medical care and support services for continuity of care.
• Monitor treatment adherence and documentation according to program rules.
• Enter documentation accurately into electronic systems and reports.
• Collaborate with an interdisciplinary team to support patient-centered outcomes.
• Conduct rapid HIV testing in office and community settings.
• Participate in outreach activities and patient engagement initiatives.
• Complete required continuing education per program regulations.
