Job summary

Virginia Commonwealth University (VCU) School of Medicine is seeking a Ryan White Case Manager to coordinate and guide services to improve access to HIV care for clients at the Infectious Diseases Clinic. The role includes case management, data entry in CAREWare/EPIC, and quality improvement activities within the MBU Pediatrics department.

The position emphasizes comprehensive assessments, care planning with multidisciplinary teams, and strong documentation to support eligibility and referrals.

Qualifications

• 3 years of Case Management experience in a healthcare delivery system.
• Knowledge of Medicare, Medicaid, and other state or federally funded assistance programs for eligible individuals.
• Knowledge of HIPAA and ability to implement privacy and confidentiality practices.
• Excellent customer service and interpersonal skills.
• Excellent time management and organizational skills.
• Ability to demonstrate superior teamwork behaviors.
• Ability to work independently.
• Proficient in MS Office and data entry programs.
• Sensitive to confidential health information and HIV/AIDS-related issues and controversies.
• Outstanding communication skills, oral and written.
• Demonstrated ability to work with diverse populations with respect and professionalism.

Responsibilities

• Screen for initial eligibility for the Ryan White grant program and annual renewals.
• Initial assessment and identification of service needs, referral to RW support services, and determination of referral to MCM.
• Provide direct social work/case management services to clients living with HIV/AIDS and their families.
• Formulate a plan of care with multidisciplinary teams to promote wellness.
• Maintain documentation of all interactions per standards.
• Respond to referrals from community organizations and health departments.
• Ensure eligibility for grant-funded services is up-to-date.
• Assist with referral processes and follow up on appointments.
• Collect data and enter CAREWare data per policies.
