Aventura at Carriage is seeking an Admissions Coordinator to manage the admissions process for skilled nursing and rehabilitation communities. You will collaborate with hospitals, families, and staff to ensure timely, accurate admissions while maintaining regulatory compliance.

The role emphasizes strong customer service, organization, and coordination across two facilities, with in-person work and opportunities for professional development.

Qualifications

• Experience in admissions, healthcare marketing, or long-term care preferred.
• Knowledge of Medicare, Medicaid and insurance processes preferred.
• Excellent verbal, written, and presentation skills.

Responsibilities

• Oversee admissions process from referral to resident admission with accuracy.
• Coordinate tours for prospective residents and families.
• Review referrals and collaborate with interdisciplinary team for admission decision.
• Maintain relationships with hospitals, physicians, and discharge planners.
• Ensure timely completion of admission paperwork and documentation.
• Keep admissions records and reports up to date.
• Support census development and occupancy growth initiatives for two facilities.
• Serve as liaison between referral sources, residents, families, and staff.
• Communicate decisions and next steps promptly and professionally.
• Coordinate admissions activities across both locations while ensuring HIPAA compliance.
