Job summary

Aventura at Shiloh is seeking an Admissions Coordinator to manage the admissions process for new residents entering our skilled nursing and rehabilitation community. You will coordinate referrals, review eligibility, schedule tours, and ensure timely completion of paperwork while delivering exceptional customer service to families and referral sources.

In this role you will collaborate with an interdisciplinary team, maintain accurate records in the EMR and reporting systems, and support census

Qualifications

• Experience in healthcare admissions, marketing, or long-term care preferred.
• Knowledge of Medicare/Medicaid and insurance processes preferred.
• Strong verbal, written, interpersonal, and presentation skills.
• Excellent organizational and time-management abilities.
• Ability to manage multiple priorities in fast-paced healthcare environment.
• Proficient with EMR systems and MS Office.
• Ability to work independently while collaborating with interdisciplinary team.

Responsibilities

• Manage admissions process from referral to resident admission.
• Review referrals and determine admission appropriateness with the interdisciplinary team.
• Coordinate tours for prospective residents and families.
• Build and maintain relationships with hospitals, physicians, case managers, discharge planners, and community partners.
• Ensure admission paperwork and documentation are completed accurately and on time.
• Maintain accurate admissions records, reports, and tracking systems.
• Support census development and occupancy growth initiatives.
• Serve as liaison between referral sources, residents, families, and facility staff.
• Communicate admission decisions and next steps timely and professionally.
• Coordinate follow-up with prospective residents, families, and referral sources.
• Ensure compliance with federal, state, facility, and healthcare regulations.
• Maintain confidentiality and HIPAA compliance.
• Provide exceptional customer service throughout admissions and transition process.
