Rehabilitation Center of Winter Park is seeking an Admissions Coordinator to serve as the primary liaison between hospitals, referral sources, residents, and families, ensuring a seamless admissions process while driving occupancy growth and exceptional customer service.

The role requires at least 2 years of SNF admissions experience, knowledge of Medicare/Medicaid/PDPM, and EMR system proficiency. Join a collaborative healthcare team dedicated to resident-focused care.

Qualifications

• Minimum of 2 years of admissions experience in a Skilled Nursing Facility (SNF).
• Strong understanding of Medicare, Medicaid, PDPM, HMO guidelines.
• Experience working with Electronic Medical Record (EMR) systems.
• Knowledge of CMS regulations and DOH requirements.
• Excellent communication, organizational, and relationship-building skills.
• Ability to multitask in a fast-paced healthcare environment.

Responsibilities

• Coordinate and facilitate the admissions process from referral through admission.
• Collaborate with hospitals and referral sources to ensure smooth transitions.
• Develop and maintain relationships with referral partners and community organizations.
• Support marketing and outreach by attending networking events.
• Educate prospective residents and families on services, policies, and procedures.
• Verify insurance coverage and authorization requirements prior to admission.
• Maintain knowledge of Medicare, Medicaid, and payer guidelines.
• Monitor admissions activity and prepare monthly reports on census growth.
• Identify barriers and work with leadership on solutions.
• Communicate admission details with the interdisciplinary team for continuity of care.
