Aventura at Carriage in Ohio is seeking an experienced Admissions Coordinator to oversee the admissions process for two nursing home facilities. You will coordinate referrals, tours, and timely admissions while building strong relationships with hospitals, physicians, discharge planners, residents, and families.

This role emphasizes compliance with federal and state regulations, HIPAA, and accurate documentation.

Qualifications

• Bachelor's degree in Healthcare Administration, Business, Marketing, or a related field preferred.
• Experience in admissions, healthcare marketing, business development, customer service, or long-term care preferred.
• Knowledge of Medicare, Medicaid, managed care, and insurance processes preferred.

Responsibilities

• Manage the admissions process for two nursing home facilities from referral through resident admission.
• Coordinate tours for prospective residents and their families.
• Review referrals and collaborate with the interdisciplinary team to determine admission appropriateness.
• Develop and maintain strong relationships with hospitals, physicians, case managers, discharge planners, and community partners.
• Ensure timely completion of admission paperwork and required documentation.
• Maintain accurate admissions records, reports, and tracking systems.
• Support census development and occupancy growth initiatives for both facilities.
• Serve as a liaison between referral sources, residents, families, and facility staff.
• Communicate admission decisions, updates, and next steps in a timely and professional manner.

This position is responsible for coordinating admissions across two nursing home facilities and is ideal for a professional who enjoys building strong relationships, coordinating care transitions, supporting census growth, and providing exceptional service to residents, families, and healthcare referral partners.

The Admissions Coordinator oversees all aspects of the admissions process, from the initial referral and inquiry through resident admission. This position works closely with hospitals, healthcare providers, residents, families, and facility teams to facilitate timely admissions while supporting census development and maintaining compliance with applicable regulatory requirements.

Qualifications

• Previous experience in admissions, healthcare marketing, business development, customer service, or long-term care preferred.
• Knowledge of Medicare, Medicaid, managed care, and insurance processes preferred.
• Excellent verbal, written, interpersonal, and presentation skills.
• Strong organizational and time-management abilities.
• Ability to effectively manage multiple priorities across two facilities.
• Proficiency with electronic medical record (EMR) systems and Microsoft Office applications.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Bachelor's degree in Healthcare Administration, Business, Marketing, or a related field preferred.
