Job summary

Aventura at Shiloh is seeking an experienced, organized Admissions Coordinator to manage the admissions process and help ensure a smooth and welcoming transition for residents entering our skilled nursing and rehabilitation community.

This role enjoys building relationships, coordinating multiple priorities, supporting families, and working closely with healthcare professionals to facilitate timely admissions.

Qualifications

• Healthcare admissions, healthcare marketing, business development, customer service, or long-term care experience.
• Knowledge of Medicare, Medicaid, managed care, and insurance processes.
• Strong verbal, written, interpersonal, and presentation skills.
• Excellent organizational and time-management abilities.
• Ability to manage multiple priorities in a fast-paced healthcare environment.
• Proficiency with EMR systems and Microsoft Office.

Responsibilities

• Manage the admissions process from initial referral and inquiry through resident admission.
• Review referrals and collaborate with the interdisciplinary team to determine admission appropriateness.
• Coordinate tours for prospective residents and their families.
• Build and maintain strong relationships with hospitals, physicians, case managers, discharge planners, and community partners.
• Ensure admission paperwork and required documentation are completed accurately and on time.
• Maintain accurate admissions records, reports, and tracking systems.
• Support census development and occupancy growth initiatives.
• Serve as a liaison between referral sources, residents, families, and facility staff.
• Communicate admission decisions, updates, and next steps in a timely and professional manner.
• Coordinate follow-up with prospective residents, families, and referral sources.
• Ensure compliance with applicable federal, state, facility, and healthcare regulations.
• Maintain confidentiality and follow HIPAA requirements.
• Provide exceptional customer service throughout the admissions and transition process.
