Job summary

Qualifications

• A high school degree or GED is required.
• A bachelor’s degree in social work or clinical counseling is required.
• A master’s degree in social work or clinical counseling is required.
• An active LISW or LPCC license in Ohio is required.

Responsibilities

• Monitors timeliness, accuracy and follow-up of referral specialist
• Ensures quality and timeliness in written and verbal communication with clients and community partners
• Responsible for performance management, recruitment, onboarding, and team development, including progressive discipline when necessary
• Provides routine support and assistance to employees through conferences and problem solving
• Leads agency-wide collaboration and innovation to support desired outcomes
• Responsible for implementation and oversight of organizational policies, procedures, and processes
• Participates in outreach activities such as follow-up calls
• Maintain compliance with HIPAA and reporting requirements
• Utilizes databases and EHR systems for documentation
• Travel within designated service areas as needed
• All other duties as assigned
