Qualifications

Responsibilities

• Monitors timeliness, accuracy and follow-up of referral specialist.
• Ensures quality and timeliness in written and verbal communication with clients and community partners.
• Responsible for performance management, recruitment, onboarding, and team development, including progressive discipline.
• Provides routine support and assistance to employees through conferences and problem analysis.
• Leads agency-wide collaboration and innovation to support outcomes.
• Responsible for implementation and oversight of organizational policies, procedures, and/or processes.
• Trains and participates in outreach and follow-up calls as necessary.
• Maintain compliance with HIPAA and mandated reporting.
• Utilizes databases and EHR systems for accurate client documentation.
