Qualifications

• Bachelor's Degree or non-licensed master level clinician required, preferably with a degree either being in behavioral health or human services.

Responsibilities

• Assess and coordinate case management activities with members to evaluate medical needs and wellness goals.
• Conduct telephonic and face-to-face assessments, planning and implementation of case management actions.
• Review prior claims and eligibility to inform care planning and program referrals.
• Collaborate with supervisors and multidisciplinary teams to address barriers and ensure goals are met.
• Use clinical judgment to reduce risk factors and support member functionality.
