Responsibilities

• Evaluate member needs and eligibility and coordinate case resolution.
• Identify high risk factors and refer to clinical case management as needed.
• Coordinate and monitor care plan activities and progress.
• Collaborate with physicians, case managers and health programs to overcome barriers.
• Escalate quality of care issues through established channels.
• Engage with members to promote healthy lifestyle changes.
