This full-time telework role requires 50-75% travel in the Metro East/Southern Illinois area, standard business hours, and a strong foundation in behavioral health or human services.

Qualifications

• 2+ years in Behavioral Health or related field.
• Willing to travel 50-75% in the Metro East, IL area.
• Electronic documentation experience and Microsoft Office proficiency.
• Reliable transportation required.

Responsabilités

• Evaluate members and assess needs/eligibility; review benefit plans.
• Coordinate and monitor care plan activities and progress.
• Identify high-risk factors and referrals for care management.
• Document care in compliance with regulatory and accreditation guidelines.
