Job summary

CVS Health Corporation in Illinois seeks a Case Management Coordinator to facilitate healthcare outcomes for members through care coordination, support and education using care management tools.

This full-time role requires 40 hours weekly with 50-75% travel within Lake County IL. A Bachelor's or related Master in behavioral health or human services and 2+ years of case management experience are required to provide coaching and support for informed health decisions.

Qualifications

• Bachelor's degree or master level clinician in behavioral health or human services is required.
• Minimum 2 years of experience in behavioral health, social services or human services field.
• Minimum 2 years of case management experience.
• Willing and able to travel up to 50-75% in Lake County IL and surrounding areas.
• Reliable transportation; mileage reimbursed per policy.

Responsibilities

• Evaluate referred member’s needs/eligibility using care management tools and data; recommend case plan.
• Identify high risk factors and service needs; refer to clinical case management or crisis intervention as needed.
• Coordinate and monitor care plan activities; ensure progress toward goals.
• Consult with case managers, supervisors, Medical Directors to overcome barriers and optimize outcomes.
• Escalate quality of care issues through established channels and document actions.
