Job summary

UAB Medicine is seeking an Access to Care Coordinator to support clients in navigating health services, assessing needs, and connecting them with resources. You will help schedule medical appointments, accompany clients when needed, and ensure accurate documentation of services and outcomes.

You will build strong connections with external referral sources and internal teams while maintaining detailed records to support continuity of care and service improvement.

Qualifications

• High School diploma or equivalent with 2 years’ experience, or an Associate/Bachelor’s degree, or 4 years of applicable job experience.
• Experience coordinating access to health-related services and guiding clients to resources.
• Strong communication, counseling, and record-keeping skills.
• Ability to build relationships with external referral sources and internal teams.

Responsibilities

• Identify potential clients, complete initial needs assessment, and follow up to guide to health services.
• Provide informal counseling, education, and social support; assist with scheduling appointments and accompanying clients.
• Develop and maintain relationships with referral sources and internal contacts.
• Maintain accurate client records documenting contacts, services, and outcomes.
