Job summary

UAB Medicine is seeking a Care Coordination Assistant to support the RN Care Coordinator, RN Case Manager, and Social Worker in ensuring patients receive needed resources across the care continuum. The role involves clerical tasks, paperwork, and coordinating post-acute transportation and lodging to support smooth transitions.

The position emphasizes CMS discharge planning, collaboration with Utilization Management for regulatory letters, and accurate medical record documentation.

Qualifications

• High School diploma or equivalent required.

Responsibilities

• Assist with clerical tasks and paperwork.
• Fill out patient assistance program applications.
• Arrange post-acute transportation and lodging needs.
• Coordinate resources for patients across the care continuum.
• Ensure CMS Conditions of Participation discharge planning are met.
• Partner with Utilization Management to deliver regulatory letters and enter authorization information into the medical record.
