Job summary

UAB Medicine is seeking an Access to Care Coordinator to support clients in navigating health services, assessing needs, and connecting them with resources. You will help schedule medical appointments, accompany clients when needed, and ensure accurate documentation of services and outcomes.

You will build strong connections with external referral sources and internal teams while maintaining detailed records to support continuity of care and service improvement.

Qualifications

• High School diploma or equivalent with 2 years’ experience, or an Associate/Bachelor’s degree, or 4 years of applicable job experience.
• Experience coordinating access to health-related services and guiding clients to resources.
• Strong communication, counseling, and record-keeping skills.
• Ability to build relationships with external referral sources and internal teams.

Job Description

Coordinate access to health-related services by assisting clients with utilizing availableresources.

• Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services.
• Provide informal counseling, education, and social support to clients. Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications.
• Develop and maintain strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members).
• Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.
