Qualifications

• Bachelor’s Degree in Physical Therapy is required.
• Current State of Illinois Physical Therapist licensure.
• For Physical Therapist II, minimum of 2 years of work experience.
• A valid driver’s license is required if duties include offsite locations.
• American Heart Association BLS CPR certification.

Responsibilities

• Provides direct physical therapy services to patients.
• Evaluates patient’s physical and functional status and develops individualized treatment plans.
• Maintains current knowledge of PT techniques and equipment and applies evidence-based practices.
