Job summary

TruHealth, part of American Health Partners, seeks an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will collaborate with a team to deliver personalized, coordinated health care that reduces hospital visits.

You will exercise independent judgment with supervision when appropriate, and work closely with facility partners and health plan leadership to implement our national model of care in compliance with regulations.

Qualifications

• APRN or PA license required (FNP/AGNP/AHACNP/PA).
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• MS Office (Word/Outlook/Excel) proficiency.

Responsibilities

• Assess patients’ medical and mental health needs and develop a care plan.
• Ensure compliance with state, federal and local clinical regulations.
• Prescribe medications and order labs, diagnostic procedures and consultations.
• Monitor patient compliance and response to treatment and adjust plans.
• Collaborate with RN case managers as part of an integrated care team.
