Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with geriatrics focus. This position offers opportunities to improve outcomes for long-term care residents by collaborating with a team of providers and case managers.

Our providers work primarily within nursing homes or assisted living communities with residents enrolled in Medicare Advantage plans.

Qualifications

• Master's degree or higher in nursing or related field.
• APRN/PA license required (APRN or PA credential).
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Electronic Health Records experience.
• Working knowledge of Microsoft Office (Word, Outlook, Excel).

Responsibilities

• Assess patients’ medical and mental health needs and create a proactive care plan.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor treatment response and adjust plans as needed.
• Collaborate with RN case managers as part of an integrated care team.
