Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with geriatrics focus to join our team caring for residents in long-term care settings. You will work with a collaborative team to improve outcomes, manage acute and chronic conditions, and support care plans aligned with Medicare Advantage populations.

Applicants must hold APRN or PA licensure, a master's degree or higher, and at least 3 years in geriatric clinical nursing or rehab, plus EHR proficiency and strong communication.

Qualifications

• APRN/PA licensure is required.
• Masters degree or higher in nursing or related field.
• At least 3 years of geriatrics clinical experience.

Responsibilities

• Assess patients and create proactive care plans.
• Prescribe medications and order labs and tests.
• Collaborate with RN case managers and supervising physician.
• Ensure compliance with local, state, and federal regulations.
