AltaMed seeks a PACE Social Work Supervisor to oversee the social work team at the designated site. You will coordinate assessments, care planning, and family conferences while guiding staff performance and ensuring regulatory compliance.

Role involves collaboration with interdisciplinary teams, onboarding new hires, and supporting end-of-life planning. A master’s degree in social work and a valid BLS certification are required; LCSW is preferred.

Qualifications

• Master’s degree in social work (MSW) from an accredited university is required; LCSW preferred.
• Minimum of 2 years in a supervisory position in health-related or human services field.
• Minimum of 2 years in case management/social advocacy/care planning with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA or Red Cross guidelines.

Responsabilités

• Provide administrative oversight and supervision of direct reports at the PACE site.
• Coordinate routine assessments and care plans; support family conferences and referrals.
• Ensure compliant documentation and onboarding of new hires; evaluate performance.
• Offer guidance on participants’ behavioral health concerns and end-of-life planning.
• Collaborate with site leaders on safety, staffing, and budgets; cover SW roles as needed.
