AltaMed Health Services in Pico Rivera, CA is seeking a Social Work Supervisor to oversee PACE site social work staff, promote interdisciplinary collaboration, and ensure compliant, person-centered care planning. Qualifications

• Master's degree in social work (MSW) from an accredited university is required. LCSW is preferred.
• Minimum of 2 years' experience in a supervisory position in a health-related or human services field.
• Minimum of 2 years of experience in case management/social advocacy/care planning with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines is required.

Responsibilities

• Coordinate routine assessments and care plans for participants.
• Assist with family conferences and community referrals.
• Monitor team member documentation and onboarding/orientation of new hires.
• Provide supervision, competency evaluations, and performance feedback.
• Offer guidance on behavioral health concerns and life planning in a person-centered approach.
