Job summary

AltaMed is seeking a PACE Social Work Supervisor passionate about making a difference in community health. This position offers the opportunity to oversee and support a dedicated team while promoting individualized care planning.

You will take part in routine assessments, onboarding new hires, and monitoring overall compliance. The ideal candidate will have a Master's degree in social work, supervisory experience, and a strong understanding of geriatric care.

Qualifications

• Minimum of 2 years' experience in a supervisory position required.
• Experience in case management/social advocacy with the geriatric population preferred.
• A valid BLS certification or higher is required.

Responsibilities

• Oversee administrative tasks and supervision of social work team.
• Promote collaboration and individualized care planning.
• Monitor team member documentation and onboarding of new hires.
