Qualifications

• Minimum 2 years experience in case management/social advocacy/care planning with geriatric population preferred.

Responsibilities

• Provide administrative oversight and supervision of direct reports at the assigned site.
• Coordinate routine assessments and care plans; assist with family conferences and referrals.
• Mentor and evaluate team members; onboard new hires; ensure regulatory compliance.
• Guide behavioral health support and life planning considerations for participants.
• Collaborate with site leaders on safety, staffing, and program budgets.
