Qualifications

• Master's degree in social work (MSW) is required; LCSW preferred.
• Minimum 2 years' supervisory experience in a health-related or human services field.
• Minimum 2 years in case management/social advocacy with geriatric population preferred.
• Bilingual abilities (Spanish/English, Chinese/English, or other) strongly preferred.
• Valid BLS certification or higher required.

Responsibilities

• Oversee administrative oversight and supervision of the PACE social work team.
• Coordinate routine assessments and care plans; assist with family conferences and referrals.
• Monitor team member documentation; onboard new hires and conduct evaluations.
• Provide guidance on behavioral health, life planning, and end-of-life care; ensure regulatory compliance.
