AltaMed Health Services in Los Angeles, CA is seeking a Social Work Supervisor for the PACE program. The role oversees the social work team, coordinates assessments, care plans, and family conferences to ensure person-centered care and regulatory compliance.

Requirements include an MSW from an accredited university, at least 2 years in a supervisory health or human services role, geriatric case management experience, BLS certification, and bilingual abilities preferred.

Qualifications

• Master's degree in social work (MSW) from an accredited university is required.
• Minimum of 2 years' experience in a supervisory position in a health-related or human services field.
• Minimum of 2 years of experience in case management/social advocacy/care planning with the geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Oversee coordination of routine assessments and care plans.
• Supervise master-level social workers and PACE team members.
• Support family conferences and community referrals.
• Monitor team documentation and onboarding/orientation of new hires.
• Ensure compliance with state and federal social work regulations and provide guidance on behavior health considerations.
