AltaMed Health Services is seeking a PACE Social Work Supervisor to provide oversight and guidance at our PACE site. This role includes supervising social workers and interdisciplinary team members while promoting effective collaboration within the team.

The ideal candidate will hold a Master's degree in social work, have supervisory experience, and possess strong case management skills with the geriatric population. Knowledge of regulatory requirements is essential for compliance.

Qualifications

• Minimum of 2 years' experience in a supervisory position.
• Experience in case management/social advocacy/care planning with the geriatric population preferred.
• Valid BLS certification or higher required.

Responsibilities

• Coordinate routine assessments and care plans.
• Monitor team member documentation and provide evaluations.
• Guidance related to behavioral health concerns for participants.
