AltaMed Health Services Corp. in Anaheim, CA is seeking a PACE Social Work Supervisor to oversee the social work team at the PACE site, coordinating assessments, care planning, and family conferences to support participant-centered care.

The role requires an MSW, 2+ years of supervisory experience, bilingual capabilities, and a valid BLS certification; offers competitive salary and comprehensive benefits fostering professional growth.

Qualifications

• Master's degree in social work (MSW) from an accredited university.
• LCSW is preferred.
• Minimum of 2 years' supervisory experience in a health-related or human services field.
• Minimum of 2 years in case management/social advocacy/care planning with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsabilités

• Coordinate routine assessments and care plans for PACE participants.
• Assist with family conferences and community referrals.
• Monitor team documentation and onboarding of new hires.
• Provide guidance on behavioral health and end-of-life care.
• Ensure regulatory compliance and site safety in collaboration with leaders.
