Job summary

AltaMed Health Services Corporation in Lynwood seeks a Social Work Supervisor at the PACE site to oversee a team of master level social workers and PACE staff. You will coordinate assessments and care plans within an interdisciplinary framework, and support family conferences and community referrals.

The role requires a MSW with at least 2 years in supervision and geriatrics, plus strong documentation, regulatory know-how, and bilingual abilities.

Qualifications

• A master's degree in social work (MSW) from an accredited university is required.
• LCSW is preferred.
• Minimum of 2 years' supervisory experience in a health-related or human services field.
• Minimum of 2 years of case management/social advocacy/care planning with the geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language is strongly preferred.
• A valid BLS certification or higher is required.

Responsibilities

• Provide administrative oversight and supervision of direct reports at the assigned PACE site.
• Coordinate routine assessments and care plans, and support interdisciplinary collaboration.
• Assist with family conferences and community referrals.
• Monitor team member documentation, onboarding, and orientation of new hires.
• Conduct routine competency and work performance evaluations.
• Guide on behavioral health concerns and end-of-life care planning.
• Ensure compliance with state and federal regulatory requirements.
• Collaborate with site leaders on safety, staffing changes, and program budgets.
• May cover SW department roles as needed.
